The Los Angeles Superior Court requires that you submit a Civil Case Cover Sheet (Judicial Council Form CM -010) when submitting your case initiating documents. 

The court also requires that you attach a Civil Case Cover Sheet Addendum and Statement of Location (Local Form LACIV 109) to the Civil Case Cover Sheet. 

These documents must be combined into one PDF as a single document. The Civil Case Coversheet should come first, followed by the Addendum and the Statement of Location.
